(tmm-prompt): Major cleanups. Handle pop-menu case nicely.
Arg BIND renamed to MENU.
Look at MENU to decide whether it is a keymap.
Arg IN-POPUP now used only in recursive call.
Use "Menu bar" as the default menu name.
Delete some debugging code.
