Allow numeric headings. (adaptive-fill-first-line-regexp): New variable. (fill-context-prefix): Use adaptive-fill-first-line-regexp. If prefix from first line seems unreasonable, use whitespace instead. Reject it entirely only if it is a paragraph-starter.
