(diff-hunk): Use it.
(diff-ignore-whitespace-hunk): Rename diff-refine-ignore-spaces-hunk.
(diff-refine-change): Rename from diff-fine-change. Change it.
(diff-refine-preproc): Rename from diff-fine-highlight-preproc.
(diff-refine-hunk): Rename from diff-fine-highlight.
