Add skeleton support for help strings on menus.
(add_menu_item): Native checkbox and radio support added, but not yet
enabled due to bugs.
(push_menu_item): Add parameters type, selection and help.
Callers updated.
Formatting changes to reduce unnecessary diffs with xmenu.c.
