(add-change-log-entry): Skip copyright notice
and copying permission notice at start of file, if any.
Make use of terms "entry" and "item" accord with Emacs manual.
Simplify the logic for moving point while entering or creating
an entry and then an item.
(add-change-log-entry-other-window): Doc fix.
