(__malloc_size_t) [DOUG_LEA_MALLOC]: Don't redefine it.
(__malloc_size_t) [!DOUG_LEA_MALLOC]: Define unconditionally as
size_t.
(__malloc_extra_blocks): Declare as __malloc_size_t.
