Remove some obsolete appt.el code.
* lisp/calendar/appt.el (appt-issue-message, appt-visible, appt-msg-window):
Remove options, obsolete since 22.1.
(appt-display-format, appt-display-message): Remove
backwards-compatibility code.
(appt-check): No longer check appt-issue-message.
(appt-make-list): No longer autoload it. Doc fix. No longer
activate the package.
