(PTY_ITERATION, PTY_NAME_SPRINTF, PTY_TTY_NAME_SPRINTF):
Defined to use all the available PTYs in an ELF environment which
uses the SVR4 mechanisms; grantpt(), unlockpt() and ptsname().
(FIRST_PTY_LETTER): Deleted.
