- init & check SDL availability at preinit() (fixes -vo sdl,mga)
- moved global init/uninit code to preinit/uninit
- -fixed-vo support, still buggy (leak some memory and after a while it
permanently kills xv, only X restart helps... or just nvidia sucks again)
